HOUSING Support Supplemental WITH SERVICES - INDEPENDENT (HWS-I): THE HOUSING WITH SERVICES/LONG TERM HOMELESSNESS (LTH)
At Metropolitan Human Resources, we recognize that homelessness is a critical issue in our community. We are committed to addressing homelessness through our Housing with Services – Independent (HWS-I): Long-Term Homelessness Program. This program provides affordable housing units and supportive services to individuals who have experienced long-term homelessness. Our HWS-I program has several key components:
- Affordable Housing: We offer affordable housing units that are safe, secure, and well-maintained. Our housing units are designed to meet the needs of individuals who have experienced long-term homelessness, providing a stable and supportive environment as they work to rebuild their lives.
- Supportive Services Our HWS-I program provides a range of supportive services to help residents maintain their independence and build a strong foundation for their future. Our services include case management, healthcare services, financial assistance, and life skills training to support residents in maintaining permanent housing.
- Personalized Care: We offer personalized care plans to ensure that each resident receives the support and assistance they need. Our team works closely with residents to develop a care plan that is tailored to their unique needs and goals, including support for mental health and substance abuse recovery.
- On-Site Staffing: Our HWS-I program is staffed by a team of dedicated professionals who are available to provide assistance and support to residents. Our staff includes case managers, social workers, and other healthcare professionals who are trained to provide personalized care and support.
- Preventative Measures: We take preventative measures to ensure that our residents do not become homeless again. These measures include education to prevent reoccurrence of homelessness, rental assistance, and job readiness training. At Metropolitan Human Resources, we are committed to providing a safe and supportive environment for individuals who have experienced long-term homelessness. Our HWS-I Long-Term Homelessness Program provides affordable housing and supportive services to help our residents rebuild their lives and achieve their full potential.
How does HWS-I Program plan to prevent recidivism?
Metropolitan Human Resources’ HWS-I Program for Long-Term Homelessness takes several preventative measures to prevent recidivism, including:
- Education: Residents receive education and training to help them maintain their housing and avoid future homelessness. This includes financial literacy training, job readiness training, and life skills training.
- Rental Assistance: Financial assistance is provided to residents to help them maintain their housing. This assistance includes rent payments, utility bill payments, and help with other basic needs.
- Case Management: Residents receive ongoing case management to help them maintain their housing, access resources, and achieve their goals. This includes regular check-ins, goal setting, and progress tracking.
- Supportive Services: The program provides supportive services to residents to help them overcome barriers to maintaining their housing. This includes healthcare services, substance abuse treatment, mental health services, and other supportive services.
