Housing Stabilization Services
Metropolitan Human Resources:Â Housing Stabilization Services (HSS) is a new Home and Community based Service (HCBS) that supports people with disabilities in finding housing; setting a basic living budget; creating natural community supports by connecting with landlords and neighbors; and assisting people with understanding the rules and responsibilities of a lease.Â
Goals of HSS is to:
- Support an individual’s transition into housing
- Increase long-term stability in housing in the community
- Avoid future periods of homelessness or institutionalization
We offer both Housing Transition and Housing Sustaining.
Transition services are provided to those that need help plan for, find, and move into housing. We can provide support with all the details associated with moving, including but not limited to:
- Developing a plan
- Help with the housing application process
- Organization
- Researching housing options
- Applying for housing
- Ensuring adaptive accommodations are in-place
- Coordinating details of a move (lease details, moving truck, supplies, etc.)
Sustaining services are provided to those that need help maintain living in their current housing. The goal of this service is to support people to stay living where they want to! We are here to help with:
- Education and training to develop good relationships with landlords, property managers, neighbors, and roommates
- Support in understanding and applying for housing-related benefits
- Transportation support
- Assistive technology
- Supporting arrangement of adaptive housing accommodations
- Navigating Economic Assistance
- Connecting individual with a Community Living Support (CLS) worker if transitioning from a group setting to an individual living arrangement
To qualify for HSS, you must meet the following criteria:
- 18 years of age and receiving Medical Assistance
- Have a disability or a disabling condition
- Be experiencing housing instability
- Have a need for services due to limitations caused by individual disability
Note: Admission into Metropolitan Human Resources HSS program is subject to State of MN Department of Human Services (DHS) Eligibility Verification Review.
Documents needed for DHS Eligibility Verification Review:
- Professional statement of need, OR
- Medical opinion form, OR
- Proof of recipient of SSI or SSDI
                         AND
- MN Choices Assessment, OR
- Long term Care Coordination Plan, OR
- Coordinated Entry Assessment
HSS program started on July 20th, 2020.
Housing Access Coordination program participants may transit into the new HSS program at the time of waiver renewal.Â
